Job summary

Eliot Community Human Services, Inc. in Boston, MA is hiring a Case Manager to deliver Respite services to adults with serious mental illness. You will coordinate care, build supportive relationships, and create Individualized Action Plans to promote recovery and community living.

The role requires crisis management, strong communication, and collaboration with a multidisciplinary team. Schedule: Monday–Friday, 9am–5pm.

Qualifications

• Requires a Bachelor’s degree or Master’s degree in Psychology, Social Work or related field.
• At least 2 years of experience working with adults with mental illness.
• Valid Massachusetts drivers license.

Responsibilities

• Provide intensive and mobile case management, care coordination and support and interventions to assigned consumers either in the community or at site-based Respite.
• Coordinate and facilitate services which may include assisting consumers with managing income, insurance, other entitlements, accessing other services and community resources, and scheduling/coordination of all medical and psychiatric appointments.
• Develop IAP’s (Individualized Action Plans) for each consumer as assigned. Ensure that treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery.
• Provide support for assigned caseload, including advocacy and assistance with IAP goals.

Education

Bachelor’s degree or Master’s degree in Psychology, Social Work or related field
