Job summary

Eliot Community Human Services, Inc. in Massachusetts seeks a Case Manager to join our Respite team providing short‑term restorative services to adults with serious mental illness. You will manage 8–12 consumers, build supportive relationships, coordinate services, and promote recovery.

The role requires a Bachelor’s or Master’s degree in Psychology, Social Work or related field and at least 2 years of experience; schedule is Monday–Friday, 9am–5pm; salary range $48,000–$50,000 USD.

Qualifications

• Bachelor’s or Master’s degree in Psychology, Social Work or related field.
• At least 2 years of experience working with adults with mental illness.

Responsibilities

• Provide intensive and mobile case management, care coordination and support for assigned consumers.
• Coordinate services including income, insurance, entitlements, appointments, and community resources.
• Develop Individualized Action Plans (IAPs) and ensure goals are culturally competent and recovery‑oriented.
• Advocate for and assist with IAP goals and provide support for caseload.
