West-Tennessee-Healthcare is seeking a Case Manager to provide, coordinate, and monitor case management services for a diverse mental health caseload in Union City, Tennessee. The role operates under the Clinical Manager, with eight-hour days, five days per week, and on-call as designated by the agency.

Responsibilities include comprehensive assessments, action planning, crisis assistance, linkage to services, and meticulous documentation in line with TennCare Standards.

Qualifications

• Completes a comprehensive assessment of the consumer’s strengths, current status and goals.
• Develops an action plan with the consumer to obtain service goals.
• Provides linkage, referral and advocacy to access resources.

Responsabilités

• Completes required records and paperwork per agency guidelines.
• Interacts daily with clients with various mental health needs.
• Participates in QA&I, Utilization Management and other agency functions.
