West Tennessee Healthcare in Dyersburg, TN seeks a Case Manager to provide, coordinate and monitor case management services for a diverse mental health caseload. The role covers eight hours daily, five days weekly, with on-call needs as designated by the agency.

The position requires a behavioral health-related bachelor’s degree or RN license, a valid driver’s license, and emergency behavior management training within 90 days. Prior behavioral health experience is preferred.

Qualifikationen

• Bachelor’s degree in a behavioral health-related field or RN license.
• If another degree, 15 college hours in behavioral health or 1 year behavioral health experience.
• Valid driver’s license.
• Emergency behavior management training within 90 days.

Aufgaben

• Assess consumer strengths, status and goals to plan necessary services.
• Develop an action plan with the consumer to meet goals.
• Provide crisis assistance when needed.
• Assist with daily living to help achieve independence.
• Provide linkage, referral and advocacy to access resources.
• Complete required records and documentation.
• Meet productivity and timely paperwork standards.
• Interact daily with mental health or substance use clients; manage high-risk situations.
• Participate in QA&I and other agency activities.
