Job summary

Family & Children's Center in Wisconsin seeks a dedicated case manager to support adults with severe and persistent mental illness in the CSP program, helping them live independently and progress in their recovery.

The role emphasizes assessment, treatment planning, crisis intervention, and discharge planning, with strong coordination across health and social services, under HIPAA and DHS standards. Regular supervision and documentation are required.

Qualifications

• Bachelor’s degree in a behavioral science or related field with 1,000 hours of supervised post-degree clinical experience with chronically mentally ill persons.
• Bachelor’s in a field other than behavioral science with 2,000 hours of supervised post-degree clinical experience.
• Preferred: Master’s degree in social work, clinical psychology or psychiatric mental health nursing or equivalent with 3,000 hours outside CSP or 1,500 hours inside CSP.

Responsibilities

• Provide assessment and treatment planning for persons with severe and persistent mental illness.
• Implement services to assist clients in meeting recovery plan goals.
• Assist clients with managing psychiatric symptoms and co-occurring medical and substance use issues.
• Ensure safety needs including housing, food and access to services; provide rep payee as needed.
• Attend scheduled treatment meetings and engage with internal/external providers.
• Complete client service paperwork in a timely, thorough manner.
• Provide after-hours crisis services to CSP clients.
