Job summary

Seaport Scripps Home Health in Oceanside, CA is seeking a licensed social worker to join our interdisciplinary team delivering comprehensive home health services. You will assess psychosocial needs, develop care plans with patients and families, and coordinate with physicians and caregivers to support optimal outcomes.

The role requires a CSWE-accredited MSW (or higher) and at least one year of health care social work experience, plus valid driver’s license.

Qualifications

• Master's or doctoral degree in social work from a CSWE-accredited program.
• Minimum of one year of social work experience in health care setting.
• Licensed driver with a vehicle insured as required by state law.

Responsibilities

• Assesses and develops the individualized social work plan in partnership with the patient and family.
• Documents psychosocial and environmental factors affecting health and communicates findings to the core team.
• Provides counseling to patients, caregivers, and families and assists with community referrals.
• Coordinates with physicians and other health professionals to update the home health plan of care.
• Participates in discharge planning and case conferences as required.
