Job summary

Adaptive Home Health is seeking a Medical Social Worker (MSW) to conduct psychosocial assessments, evaluate home-based needs, and connect patients with community resources. You will collaborate with physicians and the care team to address barriers to recovery and support discharge planning.

The role requires an MSW or DS W from a CSWE-accredited program, Nevada LMSW/LCSW or state licensure, CPR/BLS, and a valid driver’s license.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work
• Nevada LMSW or LCSW license (or state licensure as required)
• Minimum 1 year of social work experience in healthcare setting (home health preferred)
• CPR certification (BLS)
• Valid driver's license and transportation
• Knowledge of community resources including housing, financial aid, mental health, and funeral planning
• Excellent oral and written communication
• Ability to work with high autonomy in field-based settings
• Flexibility to travel locally
• Fluency in English (written and spoken)

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs
• Evaluate relationship between medical needs and resources at home
• Connect patients with community resources for housing, transportation, and financial aid
• Provide counseling and education for coping with illness-related challenges
• Collaborate with physicians and care team on psychosocial factors
• Participate in Plan of Care development and evaluation
• Monitor changes in psychosocial status and response to interventions
• Participate in discharge planning for continuity of care
• Serve as resource on psychosocial needs for the team
• Supervise and evaluate Social Work Assistants where applicable
• Maintain accurate documentation including visits and case conferences
