Job summary

Oak Street Health seeks a Medical Social Worker to join our centers, delivering psychosocial assessments, advocacy, and comprehensive care coordination for high-risk patients and their caregivers to help improve health and reduce hospitalizations.

You will collaborate with interdisciplinary teams, educate patients on care options, navigate insurance benefits, and document interventions to keep the team informed.

Qualifications

• MSW degree from CSWE-accredited program.
• Licensed (or minimally licensed) in the state where OSH Center is located.
• US work authorization.

Responsibilities

• Proactively support high-need patients with assessments, telephonic check-ins, and in-person visits.
• Collaborate with patients and supports to set goals and execute care plans.
• Refer to community resources to address healthcare and psychosocial needs.
• Navigate insurance benefits with the Patient Relations Manager to optimize plan use.
• Provide longitudinal and episodic care coordination to empower patient self-management.
