You will coordinate with the care team, assist with social security, Medicaid, Medicare, and ACA enrollment, and maintain documentation to support care continuity and program reporting.

Qualifications

• Active Maryland State Social Work License required.
• MHBE Insurance Assister/Counselor Certification within 1 year of hire.
• Three years of related experience in health care, community, case management or outreach.

Responsabilités
