Job summary

Qualifications

• Bachelor’s Degree in health-related field with licensure as appropriate.
• Master's Degree in a related field.
• MHBE Insurance Assister/Counselor Certification to be obtained within one year of hire.
• Three years of related experience in a health care setting and community, case management, and/or outreach experience.

Responsibilities

• Provide a wide range of social work and medical case management services to CBHS patients and families, addressing social determinants of health.
• Perform comprehensive intake assessments and document duties.
• Coordinate referrals and linkage to resources; monitor progress and reassess.
• Facilitate applications for Social Security, Medicaid, Medicare, and ACA enrollment.
• Collaborate with multidisciplinary team to provide coordinated care and outreach.
