Job summary

Hebrew Center for Health & Rehabilitation in West Hartford, CT seeks an MDS Coordinator / Nurse Assessment Coordinator to complete and assure the accuracy of MDS assessments for residents, supporting personalized care plans and appropriate reimbursement.

You will collaborate with interdisciplinary teams, monitor MDS processes and coding, ensure regulatory compliance, and provide ongoing education to staff while delivering high-quality care.

Qualifications

• Valid state RN nursing license.
• Experience in long-term care settings preferred.
• Knowledge of MDS processes and coding.
• Familiarity with EMR and MDS 3.0.
• Understanding of PDPM and reimbursement considerations.
• Strong organizational and communication skills.

Responsibilities

• Complete and assure accuracy of MDS assessments for all residents.
• Contribute to personalized resident care plans and reimbursement.
• Monitor Case Mix Index (CMI) and regulatory compliance.
• Collaborate with multidisciplinary teams for data collection.
• Provide ongoing education to facility staff and leaders.
