Pinnacle-Health- in North Berwick, ME, is hiring a full-time, 32-hour Licensed Social Worker to coordinate admissions for short-term rehab and skilled nursing. You will manage referrals, coordinate reviews and authorizations, and build strong hospital partnerships to drive census growth.

The role requires an LSW with 2+ years in admissions or related healthcare fields, plus knowledge of Medicare/Medicaid and payer systems.

Qualifications

• Licensed Social Worker (LSW) is required.
• Minimum 2 years experience in admissions, social services, case management, discharge planning, or related healthcare field preferred.
• Solid knowledge of Medicare, Medicaid, managed care, and insurances.
• Experience with skilled nursing facility admissions, reimbursement systems, and long-term care regulations.
• Excellent communication, relationship-building, and customer service skills.
• Strong organizational skills and ability to manage multiple priorities.
• Proficiency with electronic referral platforms, EMRs, and health care software.

Responsibilities

• Triages referrals and coordinates clinical reviews to support timely admission decisions.
• Verifies payer sources and obtains authorizations with administration.
• Collaborates with nursing leadership and interdisciplinary team for clinical appropriateness and capacity.
• Serves as liaison with hospitals, case managers, discharge planners, and referral sources.
• Communicates with Social Services about bed availability and resident status.
• Supports discharge planning and transitions for skilled residents.
• Maintains referral, admission, census, and occupancy records in EMR systems.
• Develops outreach strategies to increase admissions and strengthen referrals.
