Ebenezer Ridges Care Center, 104 skilled bed nursing home, is a 5 star CMS community and part of a larger campus (Adult Day Program, Child Care, Assisted Living, Memory Care & Independnet Living).

We are a part‑time Licensed Social Worker position (afternoon .5, 40 hours/ppd). This position is responsible for admission paperwork for the 12‑bed TCU, which will expand to 24 beds in August 2015, and for a long‑term care caseload of 16. The role requires working as a team with other social workers on campus. The ideal candidate brings experience in transitional care, long‑term care and admissions, strong communication, problem‑solving skills, and a desire to become a vital part of a dedicated team on a growing campus.

Qualifications

Licensed Social Worker (LSW) is required.
