Western Reserve Area Agency on Aging (WRAAA) is seeking a Transitional Care Coach (LPN) to support members during critical transitions after hospital or facility discharge. The role emphasizes post-discharge assessments, education, care coordination, and linking patients to community resources to improve outcomes and reduce readmissions.

Qualifications

• Active, unrestricted LPN licensure.
• Experience in care transitions, care management, home health, or community-based care preferred.
• Proficiency with electronic health records and documentation systems.

Responsabilités

• Conduct post-discharge Care Transitions Assessment (in-person or telephonic).
• Provide patient education on diagnoses, meds, and self-management.
• Identify gaps in care and connect to community resources.
• Coordinate with providers and care managers to support continuity of care.
• Complete two post-discharge follow-up calls and document in system.
