Job summary

Housing Works is seeking a creative Arts therapist Licensed Master Social Worker/Case Manager for the Adult Day Health Care (ADHC) program in Brooklyn. The LMSW will provide mental health and substance use counseling, crisis intervention, and case management to clients with HIV and chronic illnesses, working with an interdisciplinary team.

The role emphasizes patient-centered community mental health and harm reduction, with supervision from Clinical leadership and ongoing data-driven practice.

Qualifications

• NYS Licensed Master Social Worker (LMSW) required.
• Caseload management experience (approx. 30–40 clients).
• Experience in community health, HIV/AIDS, or substance use settings preferred.

Responsibilities

• Manages a caseload of 30–40 active clients, including assessments and care planning.
• Provides case management, counseling, outreach, referrals, and discharge planning.
• Facilitates groups, participates in care planning, and supports crisis interventions when needed.
• Coordinates multidisciplinary services and monitors Medicaid status as appropriate.
• Presents psychosocial needs and supports interdisciplinary care planning meetings.
