Job summary

Ohio Living is seeking a Licensed Physical Therapy Assistant (LPTA) to provide therapy services in home health. You will implement the plan of care, educate patients and families, and collaborate with the Physical Therapist and care team to ensure safe, effective treatment in the patient's home.

Requirements include an Ohio LPTA license, a CAPTE-accredited degree, and English proficiency. Travel to patient homes and weekend coverage may be required; driving is involved.

Qualifications

• Associate degree in Physical Therapy Assistant Technology from CAPTE-accredited program.
• Current Ohio license to practice as a Physical Therapy Assistant (LPTA) required.
• One year of LPTA experience preferred; new grads welcome.
• Experience with geriatric and physical therapy patients preferred.
• Must read, write, speak and understand English.
• Valid driver's license and insurability required.

Responsibilities

• Implements the Plan of Care developed by the Physical Therapist per physician orders.
• Communicates with the PT, physician, and staff about patient progress and needs.
• Educates patients and families on exercises and self-care activities.
• Participates in interdisciplinary team meetings and discharge planning.
• Performs home visits to implement the Plan of Care and improve function.
• Maintains patient records in electronic medical records per standards.
