Qualifications

• Associate degree in Physical Therapy Assistant Technology from a CAPTE-accredited program required.

• One year experience as a Licensed Physical Therapy Assistant preferred; new graduates welcome.
• Experience in geriatric and physical therapy patients required.
• Must read, write, speak, and understand English.

Responsibilities

• Implements the Plan of Care developed by the Physical Therapist.
• Communicates with the Physical Therapist and other staff about care as patient conditions require.
• Collaborates with clinical team and Physical Therapist on patient assessments (OASIS).
• Educates patient and family on exercises, precautions and self-care activities.
• Participates in interdisciplinary meetings to ensure continuity of care.
• Performs home visits to guide therapy and patient independence at home.
• Maintains clean, sanitary, and safe work areas and equipment.
• Delivers PT treatments in line with productivity standards.
• Maintains current patient records using EMR per standards.
