Rehab Without Walls® Neuro Rehabilitation in Walnut Creek, CA is seeking a Licensed Clinical Social Worker (LCSW) to join our Home & Community-based Neurorehabilitation Program. This PRN position offers meaningful flexibility while allowing you to deliver impactful care in patients' homes.

The ideal candidate will possess a Master's Degree in Clinical Social Work and experience in rehabilitation settings. Responsibilities include evaluations, care coordination, and patient counseling. The hourly pay ranges from $60.00 to $70.00.

Qualifications

• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting.
• Ability to lift 50 pounds and assist patients with mobility.

Responsibilities

• Perform comprehensive evaluations of patients' social and emotional needs.
• Set measurable objectives in conjunction with the rehab team.
• Instruct patients and families in coping with social and emotional responses.
