Job summary

PALS OHIO, LLC is seeking an experienced Individual Support Coordinator in Ohio to coordinate supports for adults with developmental disabilities. You will work with an interdisciplinary team, implement Individuals Service Plans, monitor transportation and billing, and act as liaison to the Program Director while ensuring incident reporting and compliance.

This role requires a college degree or equivalent (or 5 years direct experience) and a valid driver’s license, plus CPR/First Aid and other

Qualifications

• Must have a college degree from an accredited university or equivalent.
• 2 years of field experience or at least 5 years in the field.
• Valid driver’s license with acceptable driving record.
• Criminal background verification with acceptable clearance.
• CPR, First Aid, Crisis Intervention, Behavior Support Plan certifications.

Responsibilities

• Oversee coordination of supports and activities for clients.
• Monitor incident reporting procedures and documentation.
• Coordinate transportation needs with the Transportation Coordinator.
• Ensure timely completion of program documentation and attendance logs.
• Present at weekly staff meetings and liaise with the Program Director.
