University Hospitals Avon Rehabilitation Hospital is seeking a dedicated Case Manager for inpatient rehabilitation to coordinate care, discharge planning, and IPoC documentation in collaboration with an interdisciplinary team. The role emphasizes ensuring CMS-compliant care plans and effective stakeholder communication.

Ideal candidates will hold an OH license in RN, social work, or related therapy fields, with 2+ years of inpatient case management experience and proficiency in Microsoft Office.

Qualifications

• RN, Social Work, Respiratory Therapy, Physical Therapy, or SLP license in OH required.
• Minimum 2 years of experience in social work or case management in an inpatient setting preferred; acute rehab experience preferred.
• Proficiency in Microsoft Office applications required.

Responsibilities

• Complete departmental orientation and initial/annual competencies.
• Collaborate with interdisciplinary team to identify barriers to care or discharge and develop solutions.
• Document patient care activities, including IPoC per CMS guidelines.
• Schedule family conferences and communicate discharge planning with caregivers.
• Coordinate weekly interdisciplinary team conferences to monitor treatment goals and outcomes.
• Review CMG and communicate findings to HIM team.
