Job summary

Jobtailor is seeking a Speech-Language Pathologist to provide services in home health or facility settings. You will evaluate and treat communication disorders, develop Plans of Treatment, and document outcomes as part of a multidisciplinary team.

Responsibilities include conducting diagnostic tests, counseling families, and coordinating care while adhering to ASHA standards and maintaining patient confidentiality.

Qualifications

• Be licensed and certified in SLP per ASHA standards.
• Minimum six months experience as a speech therapist or SLP.
• Current CPR certification.
• Home health experience preferred.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review Plans of Treatment and Plans of Care.
• Assess and treat speech, voice, language, hearing, and swallowing disorders.
• Administer and interpret diagnostic tests and audiologic screening.
• Document progress and report to supervisors/physicians.
• Instruct patients in alternative communication systems as appropriate.
• Counsel patients, families, and healthcare staff.
• Maintain required documentation and reports.
• Participate in care coordination and discharge planning.
• Protect patient confidentiality.
• Help patients regain strength and independence through home healthcare.
