Job summary

CommonSpirit Health is seeking a skilled Speech Language Pathologist for VNA Home Health on a PRN basis. You will assess, diagnose, and treat patients in their homes, focusing on speech, language, swallowing, and cognitive-communication goals.

You will collaborate with families and health care professionals, develop individualized treatment plans, and document progress in accordance with regulatory standards. A compassionate, patient-centered approach is essential.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active SLP license in the practicing state and CCC-SLP certification (ASHA).
• CPR/BLS certification current.
• At least 1 year of speech therapy experience in a clinical setting.
• Current driver's license.
• Compliance with federal, state, and local laws and regulations.

Responsibilities

• Provide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.
• Customize therapy strategies to meet individual patient goals and objectives.
• Maintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.
• Provide training and support for family members to assist with therapeutic exercise and strategies at home.
