Job summary

Millennium Physician Group is seeking a Primary Care Advanced Practice Provider (APRN) in Florida. The role involves delivering patient-centered care, collaborating with physicians and an interdisciplinary team, and managing acute, chronic, and palliative needs.

You will document in the EHR, support care coordination, and participate in value-based care initiatives. The position is PRN, with responsibilities across preventative care, care gap closure, and HCC documentation while ensuring safe

Qualifications

• Master's or Doctor of Nursing Practice required.
• Current NP license in the applicable state.
• National board certification (FNP/AGPCNP/AGACNP) required.

Responsibilities

• Provide total care for all assigned patients in collaboration with care team.
• Perform patient assessments and care planning, develop risk management and care plans.
• Provide acute, chronic, and palliative care within scope of practice.
• Document active medical conditions in EHR in a timely, compliant manner.
• Coach patients on therapeutic regimens and health maintenance.
