Job summary

Advocate Health's Case Management department in Charlotte, NC seeks a Master’s level Social Worker to perform psychosocial assessments and coordinate post-hospital care. The role collaborates with the Clinical Care Management team to determine appropriate care levels, supports patients and families, and coordinates referrals to community resources.

Required MSW; LCSW preferred; hospital experience and data-management skills are valued.

Qualifications

• Masters in Social Work required; LCSW preferred.
• At least 1 year hospital or health setting experience preferred.
• Experience with data management tools.

Responsibilities

• Completes psychosocial assessments for post-hospital care and the patient’s support system.
• Assists patients in coping with stress related to hospitalization, disability, or chronic/terminal illness.
• Works with the Clinical Care Management team to assess the appropriate level of care or environment setting to meet care needs across the continuum.
• Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient’s illness and its effects on their social support system.
• Provides information and education to patients on community resources and options for post-hospital care; makes referrals to community agencies as needed.
