Job summary

AccentCare in Warwick, RI is seeking a Hospice Nurse Practitioner (NP) for PRN/Part-Time coverage. You will collaborate with physicians, coordinate patient care, and provide symptom management in home-based hospice settings, ensuring comfort and quality of life for patients and families.

Applicants must hold a Master's degree in nursing, an active RI APRN license, national certification, at least 1 year of NP experience, CPR certification, a valid driver's license, and willingness to travel to

Qualifications

Graduated from an approved school of professional nursing - Masters degree.
Current and unencumbered advanced practice nurse license in the state(s) practicing.
One year of experience as an NP.
Certified by a nationally recognized advanced nurse practice organization.

Responsibilities

Initiate communication with attending physicians, other hospice staff and agencies to coordinate optimal care and resources for the patient/family.
Monitor patient progress; update the Team Director and team members; assist with discharge planning and education for patients and families.
Maintain continuity of care with the care team and case manager for patients with ongoing needs.
Prescribe medication as permitted by the state and/or state Board of Nursing.
