AccentCare, Inc. is seeking a Nurse Practitioner (Hospice) for PRN/Part-Time work in St. Louis, MO and surrounding area.

The role requires a Master’s degree in nursing, an active APN license in practicing states, and at least one year of NP experience; certification by a recognized NP organization is also required. You must be a licensed driver who can travel to all business locations. You will coordinate with physicians and hospice staff, monitor patient progress, support discharge planning,

Qualifications

• Masters degree in nursing required
• Current and unencumbered APN license in practicing state(s)
• One year of NP experience
• Certified by a nationally recognized advanced nurse practitioner organization
• CPR certification preferred
• Licensed driver able to travel to all locations
• Meet state regulations where services are provided

Responsibilities

• Coordinate with attending physicians, hospice staff, and other agencies to ensure optimal patient care
• Monitor patient progress and update care team; assist with discharge planning and teaching
• Maintain continuity of care with care team and case manager
• Prescribe medications as permitted by state boards
