Job summary

Amedisys is seeking a hospice liaison for the York County territory. This outside sales/business development role focuses on building referral networks with hospitals, nursing facilities, and physician offices to grow home health and hospice services.

You will conduct on-site visits, educate care teams, document interactions in CRM, and strive to meet admissions and payor-mix goals. Travel is expected 50% or more within the territory; experience in healthcare sales and cold calling is a plus.

Qualifications

• At least 1 year of healthcare business development/outreach experience.
• Bachelor's degree preferred but not required.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with referral sources to generate leads.
• Coordinates patient admissions with the team and completes referral-to-admission documentation.
• Conducts on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Meets admissions, product, and payor-mix goals.
• Travels within the territory and attends community events to promote Amedisys.
