Job summary

RADIAS Health is seeking a Homeless Outreach Mental Health Practitioner to deliver PATH services to unsheltered clients in Saint Paul. You will conduct outreach, screenings, and case management, developing recovery and housing goals in collaboration with clients.

The role involves a 40-hour workweek onsite, with a multidisciplinary team and opportunities for supervision. Preferred candidates have a background in behavioral health and experience serving the homeless population.

Qualifications

• Bachelor’s degree in behavioral sciences or related fields.
• 6,000 hours of supervised experience delivering mental health services.
• Experience supporting persons experiencing homelessness.

Responsibilities

• Provide PATH services (outreach, screening, case-management, re-engagement) to PATH eligible/enrolled clients, assume primary responsibilities for developing, writing, implementing, evaluating, and revising recovery/housing goals and plans in collaboration with the PATH client.
• Maintain an active caseload of 15-20 clients.
• Work within a multidisciplinary team consisting of an Advanced Nurse Practitioner, Mental Health Professional/Practitioners, Certified Peer Specialist.
• Coordinate cases with other county or community agencies including Intake, Mental Health Case Management, Adult Protection, Income Maintenance, Crisis Center, and community hospitals.
• Develop appropriate care plans with enrolled clients.
• Initial assessment to determine presence of mental illness, chemical dependency, or behavioral problems.
• Refer urgent cases where there is risk of harm to self or others to the appropriate hospital emergency room and community crisis teams.
