Job summary

RADIAS Health in Saint Paul is seeking a Mental Health Practitioner to join our Housing Navigation program, helping clients find independent housing and providing supportive services to individuals experiencing homelessness with mental illness and/or substance use disorders.

The role leads housing search, offers outreach, and maintains tenancy support, aiming to restore skills for ongoing independence in housing.

Qualifications

• Bachelor's degree in behavioral health sciences plus 2,000 hours of supportive services experience.
• No college degree with 4,000 hours supervised experience in supportive services.

Responsibilities

• Provide housing-focused services per client plans and treatment team recommendations.
• Complete Housing Focused Person Centered Plans with referred clients.
• Monitor progress and update Housing Support Plans and Crisis Plans as needed.
