RADIAS Health is hiring a Mental Health Practitioner at our Main Street Housing program. Our program provides supportive housing services collaboratively with Catholic Charities staff. RADIAS Health employees are co-located with Catholic Charities staff at the Higher Ground St. Paul site. Emphasis is placed on working with persons served in a residential setting. Our primary goals are to maintain a safe and secure milieu, to assess collaboratively with each individual what supports are needed to help maintain housing, to work with each individual to formulate a person-centered goal plan and to either provide those supports directly to each person or seek out outside providers who can more effectively assist in this manner. We work with other members of the team to assist persons served in obtaining and maintaining optimum quality of life consistent with individual needs and preferences.

***The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge. Our ranges are developed and published as good faith estimates for the position.***

Starting Salary Range: $52,797 - $55,021 + Office Benefit Package (Parking/Remote Expenses): $100/Month subject to change

Hours: Monday - Friday 1 pm-9 pm or 2 pm-10 pm, 40 hours per week

This position works 100% onsite Main Street Housing - Higher Ground St. Paul

Duties and Responsibilities:

• Using person-centered approaches, provide direct services to clients to help them maintain housing, including connecting to outside resources. Assist, teach, and support clients with activities of basic independent living skills, being a good tenant, and their rights as a renter.
• Defuse and de-escalate any potential behavior emergencies either in person or from calls received by persons served and assist in developing additional programming appropriate for the person's needs.
• Maintain records of progress for all direct and collateral contact. Documentation will include an assessment of client's ability to function and manage the symptoms of his/her illness based on observation and interaction with the client.
• Create a unique service plans for each person served.
• Assist clients with applying, maintaining, and understanding benefits, such as Cash, SNAP, HSS eligibility, medical insurance, and Social Security. Help with completing necessary paperwork, obtaining proofs, getting documents signed by Qualified Professionals, contacting County, setting up appointments, and referring to outside resources when necessary.
• Assist clients with annual housing recertifications. Help with completing necessary paperwork, obtaining proof, and meeting with property management.
• Communicate effectively and professionally with clients. Build and maintain professional relationships and boundaries with clients.
• Communicate effectively, professionally, and collaboratively with Catholic Charities, other treatment team members, county financial workers, and others in the community. Build professional relationships to maintain housing, help advocate for clients; provide property management-tenant support and mediation.
• Assist with discharge planning.
• Assist with transporting clients in the community.
• Attend and participate in all scheduled team meetings.
• Assist in the orientation of new admissions to the program care.
• Responsible for providing staff coverage and general support of persons served.
• Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
• Is familiar and maintains compliance with applicable Minnesota Statues or rules, including, but not limited to DHS, MS 253B, Maltreatment of Vulnerable Adults, Duty to Warn, Minnesota Data Practices Act.

• Bachelor's degree behavioral science or related fields; and 2,000 hours of supervised experience in the delivery of services to persons with mental illness.
