Job summary

CenterWell Home Health is seeking a licensed Speech Language Pathologist to deliver in-home evaluations and therapy, reporting to the Clinical Manager. You will assess and treat communication and swallowing disorders, develop and adjust plans of care, and coordinate with the care team to optimize patient outcomes.

In this patient-facing role, you will work under Humana's TB and driver-safety programs, with a pay-per-visit model and competitive base range.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology (ASHA) or Audiology.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects.
• Observe, record and report changes in the patient's condition.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Maintain and submit documentation as required by the Company.
