Job summary

Adventist HealthCare Home Health in Silver Spring seeks a Speech Language Pathologist (CCC-SLP) to provide therapy across acute care, outpatient, or inpatient rehab settings as needed. The role emphasizes patient-driven plans of care, therapeutic progression, and discharge planning in coordination with the care team.

The ideal candidate holds a degree in Speech-Language Pathology, has BLS, Maryland licensure, and at least some home health experience; a driver’s license is required.

Qualifications

• Bachelor's, Master's, or Doctorate in Speech-language pathology from accredited program
• 1-year home health experience preferred
• Valid driver’s license with agency insurance requirements
• Active American Heart Association Basic Life Support (BLS) certification required
• Active Maryland License

Responsibilities

• Perform initial examination and evaluation to establish impairment/diagnosis, prognosis and plan of care across rehab settings
• Provide or supervise therapy intervention aligned with exam results and plan of care
• Plan for discharge with goals and follow-up or referral
• Communicate and document all aspects of patient management with care team
• Manage schedule to accommodate census and caseload; participate in team meetings
