Job summary

Qualifications

• Bachelor's, Master's, or Doctorate in Speech-Language Pathology from an accredited program.
• 1-year home health experience preferred.
• Valid driver’s license with agency requirements.
• Active American Heart Association Basic Life Support (BLS) certification required.
• Active Maryland License.

Responsibilities

• Perform an initial examination and evaluation to establish diagnosis, prognosis and plan of care.
• Provide or supervise therapy interventions aligned with the assessment and care plan; involve patient and care team.
• Plan for discharge, with follow-up or referrals as needed.
• Communicate, coordinate and document all aspects of patient management with the care team.
• Supervise and direct rehab aides and manage caseload; attend team meetings.
