Job summary

Elara Caring is seeking a Registered Nurse - Home Health to provide high-quality patient care across home settings. You will assess patients, complete initial and ongoing evaluations, and deliver care according to physician orders while ensuring patient privacy and safety.

Qualified candidates have an active RN license and 1+ year of clinical nursing experience. Home health experience is a plus, and you must be able to lift up to 50 pounds and travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate to BSN).
• 1+ year of clinical nursing experience.
• Home Health experience preferred but not required.
• Ability to lift and move up to 50 pounds and travel between homes.

Responsibilities

• Assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Provide nursing care per plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Maintain HIPAA privacy and safety standards.
