Job summary

UT Southwestern Medical Center is seeking a Home Health RN Case Manager to provide nursing care per the client’s plan of care in the DFW area. This role includes comprehensive health and psycho-social evaluations, condition monitoring, health promotion, and coordinated services for patients in home health settings.

Requirements include a licensed RN with 2 years of RN experience and 1 year in adult IV therapy, along with Texas driver’s license and BLS certification.

Qualifications

• Graduate of accredited nursing program.
• 2 years of RN experience with one year of adult IV expertise required.
• RN license (TX or BNE compact) and Texas driver’s license; proof of insurance.
• BLS certification from AHA or ARC.

Responsibilities

• Provides direct nursing care and conducts initial and ongoing comprehensive assessments.
• Evaluates patient care outcomes and coordinates discharge planning.
• Maintains patient records, med profiles, and care plans.
• Gives patient/family instructions and teaching.
• Manages IV fluids, IV access and medications as prescribed.
• Performs phlebotomy and point-of-care testing.
• Ensures quality patient care meets regulatory standards.
• Supervises Home Health Aides and LVNs as required.
