UT Southwestern Medical Center is seeking a Home Health RN Case Manager to provide nursing care per the client’s plan of care in the DFW area. This role includes comprehensive health and psycho-social evaluations, condition monitoring, health promotion, and coordinated services for patients in home health settings.
Requirements include a licensed RN with 2 years of RN experience and 1 year in adult IV therapy, along with Texas driver’s license and BLS certification.

The Case Manager for Home Health provides nursing care in accordance with the client's plan of care, and includes comprehensive health and psycho-social evaluation, monitoring of client's condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care and other duties as deemed necessary.
