Job summary

Interim HealthCare Inc. is seeking a Registered Nurse (RN) | Case Manager to deliver in-home care to patients in the Redding, CA area.

The role emphasizes thorough assessments, wound care, medication administration, and collaboration with the care team to optimize patient outcomes. You will operate under a provider-approved plan of care, maintain detailed documentation, and support patients and families with a patient-centric approach.

Qualifications

• Must hold a current CA Registered Nurse license and graduate of an accredited nursing program.
• Two (2) years of nursing experience preferred.
• One (1) year management experience preferred.
• Must have Home Health or Hospice experience.
• Ability to pass Criminal Background Check, Physical, TB & Drug Screen.

Responsibilities

• Provide in-home direct patient care per the plan of care and nursing scope of practice.
• Assess and document patient conditions; report changes as needed.
• Administer medications, insulin, IV fluids, and document thoroughly.
• Inspect and care for wounds; assist with personal hygiene.
• Assess patient for injuries, infections, bedsores; deliver appropriate treatments.
• Collaborate with providers and health professionals to ensure outcomes.
• Participate in ongoing interdisciplinary patient assessments and QAPI.
