Job summary

Home & HealthCareManagement is the oldest home healthcare agency in Northern California, delivering comprehensive in-home care and case management for diverse clients. The team coordinates resources like housing, medical equipment, and health benefits to support participants in their communities.

Role requires in-person work in Redding with travel up to 80% to participants’ homes, and a schedule Monday–Friday 8am–5pm with lunch. Full-time position with strong focus on advocacy and documentation.

Qualifications

• Ability to communicate in English effectively in writing and verbally with colleagues, supervisors and participants.
• Knowledge of community resources including housing for low income and disabled persons.
• Experience helping people solve social determinants of healthcare problems.

Responsibilities

• Work with a Care Management Team comprising care managers who provide comprehensive case management services.
• Assist the team with finding community resources needed to meet the participant’s needs (housing, medical equipment, primary care access, food, clothing, transportation).
• Provide guidance with application processes for services such as health benefits, IHSS, CalFresh, Waiver and managed care programs.
