Job summary

Mission Healthcare is seeking dedicated registered nurses to join their team in providing home health and hospice care. You will play a critical role in assessing patient needs and developing care plans while embodying our core values of Compassion, Accountability, Respect, Excellence, and Service (CARES).

This role offers a pay range of $55-$60 an hour, dependent on experience, along with various employee benefits including medical insurance and retirement savings.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Valid driver's license and auto insurance.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Counsels the patient and family in meeting plan of care.
• Provides health care instructions to the patient.
• Identifies discharge planning needs and implements prior to discharge.
