Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse (RN) to provide patient care across home and facility settings. You will be part of an interdisciplinary team offering psychosocial assessments and coordinating services in a CHAP-certified hospice.

You may work in Case Management, Admissions, or Resource roles, traveling to patient locations with reliable transportation and maintaining EMR documentation.

Qualifications

• Current RN license in good standing in the state of practice.
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to travel across the service area with reliable transportation.
• Time management to manage visits, documentation, meetings, and urgent needs.
• Willingness to learn an electronic medical records system.

Responsibilities

• Provide care and services for a group of patients in homes or facilities.
• Participate as a contributing member of the interdisciplinary team with psychosocial assessments.
• Work in Case Management, Admissions, or Resource roles as assigned.
• Coordinate visits and documentation while traveling across the service area.
