Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist for full-time home visits in Scotland County and surrounding areas. You will provide skilled PT services to adults and geriatric clients, collaborating with a multidisciplinary team to promote health and independence.

Eligible applicants hold a North Carolina PT license with at least one year of practice and are capable of independent work, documentation via EMR, and delivering customized care plans in patients' homes.

Qualifications

• Requires current North Carolina PT license.
• Minimum of 1 year of recent PT experience.
• Graduation from an APTA/CAAHEA-accredited program; transcript or diploma required.

Responsibilities

• Make home visits to patients in designated territories.
• Perform mobility, gait, strength, balance and safety assessments.
• Develop and implement individual care plans including therapeutic exercises and gait training.
• Document observations and interventions, using EMR on a tablet.
• Educate patients and families to promote health, safety and independence.
