Qualifications

• Current North Carolina PT license.
• Minimum of one year of PT experience.
• Graduate from an APTA-approved program or AMA/CAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, balance, strength, and safety.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and adjust individualized care plans with manual therapy, gait training, and balance work.
• Educate clients and families on rehabilitative care and safety to promote independence.
• Document observations and interventions using a tablet-based EMR system.
