Job summary

AccentCare, Inc. is seeking a Physical Therapist to provide evaluation and treatment in home health settings in the Denver, CO area. This PRN position supports an interdisciplinary team and requires travel to multiple locations within the service area.

The role emphasizes patient and family education, documentation, and alignment with agency policies. A CAPTE-accredited PT degree, state licensure, CPR, and a valid driver’s license are required.

Qualifications

• Master’s degree in physical therapy (CAPTE-accredited)
• Minimum nine months experience as a physical therapist
• Licensed to practice as a physical therapist within the state of operation
• CPR certification must be maintained
• Must possess and maintain valid driver’s license and be able to travel to all business locations

Responsibilities

• Provides patient care and documents treatment per physician orders and patient needs.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with priorities.
• Educates and counsels patients and families; maintains documentation.
• Communicates patient status and plan with the team and physicians.
• Delivers treatment within agency and payer guidelines; meets productivity standards.
