Qualifications

• Active Physical Therapist license in CO.
• Reliable transportation and valid auto insurance.
• Home health experience preferred; training provided.

Responsibilities

• Perform comprehensive patient assessments and evaluations.
• Develop and implement individualized therapy treatment plans.
• Provide occupational therapy services in the patient’s home.
• Document patient progress, outcomes, and medical histories accurately.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Ensure compliance with all clinical and regulatory standards.
