Job summary

AccentCare Inc. is seeking a Physical Therapist for our Home Health team in Denver, CO.

This full-time role involves evaluating and treating patients within an interdisciplinary team, delivering patient and family education, and documenting care in line with standards and policies. The position requires a CAPTE-approved Master’s in Physical Therapy, at least nine months of PT experience, active licensure in the state, CPR certification, and a valid driver’s license to travel among our service

Qualifications

• Master’s degree in physical therapy (CAPTE approved).
• Minimum nine months experience as a physical therapist.
• Licensed to practice in the state of operation and CPR certified.
• Must be a licensed driver capable of traveling to all locations.

Responsibilities

• Provide evaluation and treatment to patients as part of the interdisciplinary team.
• Educate patients and families, document care per policy and standards.
• Develop and modify treatment plans; ensure measurable goals are achieved.
• Communicate patient status and plans with physicians and team members.
• Meet productivity standards and manage caseload efficiently.
• Perform evaluations and select diagnostic material as appropriate.
• Coordinate care across multiple office locations.
