Job summary

Ohio's Choice Home Health is seeking a licensed Physical Therapist to assess and treat patients in home health settings. You will develop treatment plans, perform interventions to improve mobility and pain, and monitor progress.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to a dependable vehicle or public transportation.

Responsibilities

• Evaluate patient's functional status and PT needs; assist in therapy plan of care.
• Provide PT services within scope of practice and plan of care.
• Observe and report patient responses to treatment to supervising nurse/physician.
• Treat patients to relieve pain and restore function.
• Instruct patient and family regarding plan of care, limitations, and progress.
• Visit patients per Plan of Treatment and submit progress notes per policy.
• Complete patient evaluations and develop PT plan of care within state guidelines.
• Report outcomes and anticipated frequency of care.
• Function in home settings without bias.
