Job summary

Optum is hiring a Physical Therapist for Home Health to assess patient needs, develop treatment plans, and deliver interventions to improve mobility and quality of life. You will collaborate with physicians, nurses and families to ensure safe, effective care in the home setting.

The role requires current PT licensure and a valid driver's license, with CPR certification preferred or obtainable within 90 days.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Develops and implements the plan of care in coordination with the care team.
• Monitors progress and documents patient responses to treatment.
• Educates patients and families about the plan of care and home exercises.
• Visits patients per Plan of Treatment and submits progress notes.
• Ensures care complies with state practice guidelines and regulations.
• Collaborates with supervising nurse and physician as needed.
