Job summary

Ohio's Choice Home Health in Ohio's LHC Group network seeks a Physical Therapist to assess and treat patients in home health. You will evaluate functional status, mobility, endurance, and pain, then help develop a plan of care and implement interventions to improve well-being.

You will document progress, communicate with the supervising nurse and physician, and collaborate with the health care team to ensure goal achievement.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists physician in therapy plan of care.
• Provides PT services within scope of practice and in coordination with health care team.
• Observes and reports patient response to treatment and any condition changes to supervisor.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes and submits progress notes per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and proposed frequency of care.
• Function effectively in any home setting regardless of demographic factors.
