Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to assess and treat patients in home-based settings. You will implement approved therapy plans to improve mobility and daily living skills, while collaborating with physicians and care teams.

Responsibilities include delivering skilled services, educating patients and families, and supporting programs like Balance and Fall Prevention within a home-care framework.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Physical therapists examine and treat patients with physical impairments using physical modalities to improve mobility and independence.
• Deliver skilled care to clients under physician orders and plan of care.
• Educate family and health care team members on care plans and home therapy routines.
