Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide in-home assessment and therapy. You will use modalities to improve mobility and independence, working with patients and families in a home setting.

The role requires state licensure, a valid driver's license, and willingness to travel to patient homes, with a focus on high-quality care and collaboration with the care team.

Qualifications

• Current state license as a Physical Therapist or relevant licensure
• Valid driver's license and automobile liability insurance
• Ability to drive and travel to patient homes in all weather conditions
• Commitment to patient-centered care and home-based services

Responsibilities

• Examine and treat patients with physical impairments using physical modalities
• Improve mobility and daily living skills for home-based patients
• Administer skilled care per physician orders and plan of care
• Educate family and health team members on care and safety
• Coordinate with Branch Director and other staff to ensure quality outcomes
